A single service will support people to be independent in their communities. We will combine NHS and Social Care services to create a single, easily accessible service that will deliver rapid response, complex care and reablement to prevent hospital admission, where possible, or allow a faster return to independence on discharge from hospital. This builds on our successful joint work delivering longer visit times for care at home, and fully paying care workers for all their hours and travelling time.
